Below you will find the different forms provided by the Office of University Scholarships and Financial Aid.

Due to privacy concerns, the office no longer accepts attachments via email. Be sure to use the Document Uploader in order to securely submit a document. Use of the Document Uploader requires a vt.edu email address or a Google account.

If failing for Maximum Timeframe (hours) you must submit both the SAP Appeal form and the SAP Academic Advisor form.

Cost of Attendance (COA) Appeal:


Cost of Attendance Appeal Form	Prior to submitting an appeal students should review the COA figures available in the financial aid award notification in Hokie SPA to determine if an appeal is necessary.

USFA University Scholarship Eligibility Appeal Form


USFA University Scholarship Eligibility Appeal Form	This form is only to appeal the loss of a renewable scholarship due to the student not meeting scholarship requirements for scholarships managed by the Office of University Scholarships and Financial Aid. This form is not for loss of college or department managed scholarships.

Global Education Financial Aid Forms:

The Global Education Office will provide these documents to students with financial aid participating in exchange or non-VT study abroad programs. For programs hosted by another university/college within the United States, please contact our office via email to determine if these documents are necessary.
